Parental Access to Medical Records for Children 12 and Older
Are you aware that in Arizona as soon as a child turns 12, the parent’s access to the child’s medical records becomes restricted?
This way a child can decide to have medical procedures such as gender surgery, abortion, birth control, etc. done without their parents knowing. The child can sign a form to give the parent proxy to all her medical files and have it be unrestricted.
