Claim Your Rights – Arizona Offers All 3 Vaccine Exemptions!

Arizona law allows for three types of exemptions to school immunization requirements: medical, religious, and personal beliefs (for kindergarten through 12th grade only). Medical exemptions require documentation from a physician or registered nurse practitioner. Religious exemptions can be claimed for childcare, preschool, and Head Start programs. Personal belief exemptions are specifically for K-12 students.
